Community Modu Living Care works exclusively with the IDD community in Oregon. IDD stands for intellectual and developmental disabilities.
An individual support plan, or ISP, is a personalized care plan designed by our direct support professionals as well as the individual's care providers to meet the specific needs and preferences of each resident under our care. It is a collaboration with healthcare professionals to develop tailored plans for residents with specific needs for living.
Community Modu Living Care prioritizes its community members by working closely with all third party providers involved with the care of our mutual candidate.
